Purpose: to evaluate the impact of dental pain and the consequences of untreated dental caries on the quality of life in children of low social-economic status aged from 8 to 10 years old. Materials and Methods: in this cross-sectional study, 230 children were submitted to a clinical examination in which the caries-pufa ratio was measured, afterward, they answered to two questionnaires: one about their quality of life (CPQ8-10) and the other about dental pain. Data were statistically analyzed through Chi-square, Mann-Whitney or Kruskal-Wallis tests to evaluate the association between the variables with impact on children's OHRQoL. To establish the existence of risk factors among variables and impact on OHRQoL a Poisson Regression model was applied. Results: dental caries (p = 0.003; PR 2.39; 95% CI 1.04­1.56), severity of untreated caries (p = 0.008; PR 2.86; 95% CI 1.13­2.00), toothache (p < 0.001; PR 2.31; 95% CI 1.64­3.27) and PUFA + pufa index (p < 0.023; PR 2.68; 95% CI 1.10­1.87) were associated with and were a predictor factor for impact on overall OHRQoL. All of these variables also had an effect over the social welfare subscale (p ≤ 0.001), whereas caries presence was also statistically linked with the emotional wellbeing subscale (p = 0.008) and dental pain with all four subscales (p ≤ 0.001). Conclusion: untreated dental caries' clinical consequences and dental pain exerted a negative influence on the quality of life of schoolchildren analyzed.(AU)


Objetivo: avaliar o impacto da dor de dente e das consequências da cárie não tratada na qualidade de vida de crianças entre 8 e 10 anos com baixos indicadores socioeconômicos. Métodos: neste estudo transversal, 230 crianças escolares foram submetidas a um exame clínico, no qual foram avaliados seus CPO/ceo (Dentes cariados, perdidos e obturados) e PUFA/pufa (Envolvimento pulpar, ulceração, fístula e abscesso). Em um segundo momento, elas responderam a dois questionários: um sobre a qualidade de vida (CPQ8-10) e outro sobre odontalgia. Os dados foram analisados estatisticamente por meio dos testes de Chi-quadrado, Mann-Whitney ou Kruskal-Wallis, para avaliar a associação entre as variáveis e o impacto na qualidade de vida relacionada à saúde oral (QdVRSO) da criança. Para estabelecer a existência de fatores de risco para impacto na QdVRSO entre as variáveis, um modelo de regressão de Poisson foi aplicado. Resultados: a presença de cárie (p = 0,003; RP 2,39; 95% IC 1,04-1,56), a severidade da cárie não tratada (p = 0,008; RP 2,86; 95% IC 1,13-2,00), dor de dente (p < 0,001; RP 2,31; 95% IC 1,64-3,27) e PUFA/pufa (p < 0,023; RP 2,68; 95% IC 1,10- 1,87) foram associados com impacto na QdVRSO, assim como também foram fatores preditores para o impacto na qualidade de vida relacionada à saúde oral. Todas as então citadas variáveis também exerceram efeito sobre a subescala do bem-estar social (p ≤ 0,001), enquanto a presença de cárie esteve estatisticamente conectada com a subescala do bem-estar emocional (p = 0,008) e a dor dentária com todas as subescalas da QdVRSO (p ≤ 0,001). Conclusão: as consequências clínicas da cárie não tratada e a dor dentária exercem uma influência negativa na qualidade de vida das crianças analisadas.(AU)

Abstract The aim of this study was to evaluate the association of malocclusion, nutritive and non-nutritive sucking habits and dental caries in the masticatory function of preschool children. A cross-sectional study was conducted with a sample of 384 children aged 3-5 years. A single examiner calibrated for oral clinical examinations performed all the evaluations (kappa > 0.82). Presence of malocclusion was recorded using Foster and Hamilton criteria. The number of masticatory units and of posterior teeth cavitated by dental caries was also recorded. The parents answered a questionnaire in the form of an interview, addressing questions about the child's nutritive and non-nutritive sucking habits. The masticatory function was evaluated using Optocal test material, and was based on the median particle size in the masticatory performance, on the swallowing threshold, and on the number of masticatory cycles during the swallowing threshold. Data analysis involved simple and multiple linear regression analyses, and the confidence level adopted was 95%. The sample consisted of 206 children in the malocclusion group and 178 in the non-malocclusion group. In the multiple regression analysis, the masticatory performance was associated with age (p = 0.025), bottle feeding (p = 0.004), presence of malocclusion (p = 0.048) and number of cavitated posterior teeth (p = 0.030). The swallowing threshold was associated with age (p = 0.025), bottle feeding (p = 0.001) and posterior malocclusion (p = 0.017). The number of masticatory cycles during the swallowing threshold was associated with the number of cavitated posterior teeth (p = 0.001). In conclusion, posterior malocclusion, bottle feeding and dental caries may interfere in the masticatory function of preschool children.

Resumen Introducción. La capacidad de la saliva para la remineralización contribuye a mantener la integridad fisicoquímica de la estructura mineral de los dientes, lo cual los protege de la aparición y evolución de la caries dentaria. Objetivo. Establecer la relación de la sialometría, la capacidad amortiguadora, la concentración de calcio y fosfato en la saliva total estimulada y en el líquido crevicular gingival, con la gravedad y la actividad de las caries en escolares. Materiales y métodos. Se seleccionaron 36 escolares de 6 años, 18 con caries (International Caries Detection and Assessment System, ICDAS, código 2 o mayor) y 18 sin caries (ICDAS, código 0). En la dentición primaria, se evaluaron la gravedad y la actividad de la caries dentaria en la superficie de oclusión de los molares y en la vestibular de los dientes anteriores, y se codificaron mediante el ICDAS-II. Resultados. Se evidenció una mayor gravedad de las caries en la superficie de oclusión que en la vestibular. Las concentraciones de calcio en la saliva y de fosfato en el líquido crevicular gingival de los dientes sanos, fueron mayores en el grupo con un código ICDAS de 2 o mayor. La concentración de calcio en el líquido crevicular gingival fue mayor en el grupo con código ICDAS 0 que en los dientes cariados del grupo con código ICDAS de 2 o mayor. Se encontró una asociación estadísticamente significativa entre la frecuencia de caries activas y la concentración de fosfato en el líquido crevicular gingival de los dientes afectados, como también entre la gravedad de las caries y la capacidad amortiguadora, por una parte, y la concentración de fosfato en el líquido crevicular gingival de los dientes afectados, por la otra. Conclusión. Se evidenció una asociación entre la caries dental, la capacidad amortiguadora y la química bucal del calcio y el fosfato.


Abstract Introduction: The remineralizing properties of saliva contribute to maintain the physical and chemical integrity of the mineral structure of teeth, which protects it from the installation and evolution of dental caries. Objective: To relate sialometry, buffering capacity, calcium and phosphate concentration in whole stimulated saliva, and in gingival crevicular fluid with school children caries severity and activity. Materials and methods: We selected 36 schoolchildren aged 6 years: 18 with caries (International Caries Detection and Assessment System, ICDAS>1 group) and 18 without caries (ICDAS=0 group). The severity and activity of dental caries were diagnosed in the primary dentition: in the occlusal surface of molars and in the vestibular of the anterior teeth by ICDAS-II. Results: Caries in occlusal surface were more severe than in vestibular surface. The concentration of calcium in saliva and phosphate in healthy teeth gingival crevicular fluid were higher in the ICDAS>1 group. The concentration of calcium in gingival crevicular fluid was higher in the ICDAS=0 group than in the decayed teeth of the ICDAS>1 group. We found a statistically significant association between the frequency of active caries andthe concentration of phosphate in gingival crevicular fluid of teeth with caries, as well as between the severity of caries with buffering capacity and the concentration of phosphate in the gingival crevicular fluid of teeth with caries. Conclusion: We found an association between dental caries with buffering capacity and buccal calcium and phosphate.

Abstract Introduction: Adequate nutrition, including intake of dietary calcium and vitamin D, is important to maintain bone health. Evidence suggests that a deficiency in micronutrients may contribute to bone loss during aging and exert generalized effects on chronic inflammation. Recently, the Dietary Inflammatory Index (DII) was developed to assess the inflammatory potential of individual diets. Our aim was to evaluate the DII in a representative sample and verify its association with low-impact fractures. Methods: Individuals from The Brazilian Osteoporosis Study (BRAZOS) database had their DII calculated. BRAZOS is an important cross-sectional epidemiological study carried out with a representative sample of men and women ≥40 years old. The research was conducted through in-home interviews administered by a trained team. Nutrition Database System for Research (NDSR) software was used to analyze data on the intake of nutrients, which were employed to calculate the DII using Statistical Analysis Software (SAS®) and Statistical Package for the Social Sciences (SPSS®) to assess its association with low-impact fractures. Results: A total of 2269 subjects had their DII score calculated using information from 24-h recall data. Males had lower DII than females (DII = 1.12 ± 1.04 vs DII = 1.24 ± 0.99, p = 0.012). Women taking statins had lower DII (DII = 0.65 ±1.14 vs DII + 1.26 ± 0.98, p = 0.002), indicating a greater potential for diet-related anti-inflammatory effects. Conclusion: Our findings suggest that women might have a pro-inflammatory diet pattern compared to men. However, we did not find any association between DII scores and low-impact fractures.

Abstract Attention-deficit/hyperactivity disorder (ADHD) is characterized by inappropriate levels of hyperactivity, impulsivity, and/or inattention. Individuals with ADHD may present limitations with regard to executive functions and performing activities that involve planning and/or attention/concentration. The aim of the study was to investigate the association between dental caries and signs of ADHD in a representative sample of schoolchildren. A representative sample of 851 schoolchildren aged seven to 12 years was randomly selected from public and private schools. Data acquisition involved a clinical dental examination for cavitated permanent and deciduous teeth using the DMFT/dmft indices. Neuropsychological evaluations, including the assessment of intelligence (Raven's Colored Progressive Matrix Test) and executive functions (Corsi Tapping Blocks tests and Digit Span test) were also performed. Parents/caregivers and teachers answered the SNAP-IV Questionnaire for the investigation of signs of inattention and hyperactivity in the family and school environment. Parents/caregivers also answered questionnaires addressing socioeconomic and socio-demographic characteristics. Descriptive analysis of the variables and Poisson regression with robust variance were performed. Parental reports of signs of inattention (PR: 1.28; p < 0.05) and hyperactivity (PR: 1.15; p < 0.05) were associated with a greater occurrence of caries. A better performance on the backward order of the Corsi Tapping Blocks tests (PR: 0.94; p < 0.05) and higher level of mother's schooling were associated with a lower frequency of caries. A better performance on executive function tasks was a protective factor against dental caries, whereas children considered inattentive and/or hyperactive by their parents had a higher prevalence rate of dental caries.

Abstract The aim of the present study was to evaluate the prevalence and impact of malocclusion on oral health-related quality of life (OHRQoL) among schoolchildren aged 8 to 10 years and their parents in Diamantina, a town in the southeast of Brazil. A cross-sectional study was conducted with a sample of 390 randomly selected children who were subjected to a clinical oral examination. The Dental Aesthetic Index was used to diagnose malocclusion and the need for orthodontic treatment. The Child Perceptions Questionnaire (CPQ (8-10)) was used to evaluate the impact of malocclusion on OHRQoL. The children's parents answered the Brazilian Economic Criterion Questionnaire for the socioeconomic classification. Data analysis involved the nonparametric Kruskal-Wallis test and Spearman's correlation coefficients. The variables were grouped into a hierarchy of categories ranging from distal to proximal determinants. Poisson regression analysis with robust variance was performed at each level to correlate the total CPQ (8-10) score with the independent variables. The prevalence of malocclusion was 78.7%. Crossbite remained significantly associated with a negative impact on OHRQoL (PR = 1.28; 95%CI:1.17-1.39; p < 0.001). The prevalence of malocclusion was high in the sample investigated and exerted a negative impact on OHRQoL.

Abstract: The aim of this study was to evaluate the correlation between the Child Perceptions Questionnaire 8 to 10 (CPQ8-10) and child-Oral Impact on Daily Performances (child-OIDP) indexes according to their total and item scores, as well as assess the discriminative validity of these assessment tools regarding dental caries and malocclusion among schoolchildren. A sample of 300 children aged between 8 and 10 years answered the questionnaires in two distinct steps. First, half of the sample (G1 = 150) answered the CPQ8-10 and the other half (G2 = 150) answered the child-OIDP. A week after, G1 answered the child-OIDP and G2 answered the CPQ8-10. Dental Aesthetic Index and WHO criteria were used to categorize malocclusion and dental caries, respectively. Descriptive analysis, Spearman's correlation and Mann-Whitney test were performed in this study. The CPQ8-10 and child-OIDP demonstrated a statistically significant and moderate correlation between their total scores. Regarding the discriminative validity, CPQ8-10 demonstrated a significant association between the "emotional status" daily activity and dental caries, and between the "eating", "sleeping", and "studying" daily activities and malocclusion. Concerning the child-OIDP, a significant difference was found only between the "social contact" activity and presence of dental caries. Both instruments were not capable of distinguishing children with and without dental caries and/or malocclusion by their total scores. However, the instruments were able to discriminate between children with and without those oral disorders in different dimensions. Thus, the CPQ8-10 and the child-OIDP demonstrated a different capacity to assess the impact on OHRQoL among schoolchildren.

A obesidade e a síndrome metabólica têm aumentado em proporções preocupantes em nível mundial. Além das consequências sistêmicas, a obesidade e suas comorbidades também têm sido relacionadas com a condição bucal. O objetivo deste estudo foi identificar a progressão de lesões cariosas, presença de biofilme dentário, fluxo salivar e presença de saburra lingual em indivíduos eutróficos, sendo avaliados em dois momentos e portadores de síndrome metabólica antes e após a cirurgia bariátrica. A amostra foi constituída por 150 indivíduos, divididos em dois grupos: grupo controle (GC: 75) e grupo obeso (GO: 75), ambos avaliados em T0 e T1, sendo GC 6 meses após a primeira avaliação e GO 6 meses após a cirurgia bariátrica. A avaliação antropométrica dos indivíduos foi realizada por meio do IMC e circunferência da cintura. Os índices ICDAS II e CPOD foram utilizados para avaliar a progressão da cárie dentária. O método de fluorescência verde (QLF- Quantitative Light-induced Fluorescence) avaliou a perda mineral nas superfícies lisas dos dentes e presença de biofilme dentário. O fluxo salivar estimulado foi avaliado em mL/min. O Índice de Saburra Lingual foi utilizado para a avaliação da presença de saburra na língua. Para análise estatística, utilizou-se os testes Qui Quadrado, Exato de Fisher, Mann-Whitney, Kruskal-Wallis e Regressão Logística Múltipla, adotando nível de significância de 5%. Os resultados mostraram que houve diferença significativas entre os grupos, GC apresentou maior número de dentes hígidos (p=0,038) e dentes sem alteração no esmalte (p=0,005) quando comparado ao GO. Ao comparar GC e GO no T1, verificou-se que houve diferença significativa em relação ao CPOD (p<0,0001), dentes restaurados (p<0,0001) e área da lesão (Ws) (p=0,045), sendo as piores condições encontradas encontradas em GO. Observou-se que houve diferença significativa entre a perda mineral e quantidade de biofilme dentário por fluorescência entre os grupos, mas não entre T0 e T1. Diante dos resultados, podese concluir que indivíduos portadores de síndrome metabólica, comparado com os eutróficos, apresentam maior número de lesões cariosas antes e após a cirurgia bariátrica. No presente estudo a cirurgia bariátrica não interferiu na progressão das lesões cariosas, todavia, destaca-se a necessidade de abordagem multiprofissional e contínua, atuando na prevenção de doenças e atenção odontológica.(AU)


Obesity and the metabolic syndrome have increased in alarming proportions at world level. Apart from systemic consequences, obesity and its comorbidities have also been related to the oral condition. The aim of this study was to identify the caries lesion progression, presence of dental biofilm, the salivary flow and tongue coating in eutrophic individuals, being evaluated in two moments and individuals with metabolic syndrome before and after undergoing bariatric surgery. The sample consisted of 150 individuals divided into two groups: Control Group (CG: 75) and Obese Group (OG): 75, both evaluated in T0 and T1; CG being evaluated six months after T0, and EG, 6 months after bariatric surgery. Anthropometric assessment of individuals was made by means of BMI and waist circumference measurement. The ICDAS II and DMFT indices were used for evaluating the progression of dental caries. The Quantitative Light-induced Fluorescence (QLF) method, for evaluating mineral loss on the smooth tooth surfaces and the presence for dental biofilm. The stimulated salivary flow was evaluated in mL/min. The Winkel tongue coating index (WTCI) was used to evaluate the presence of tongue coating. For statistical analysis, the Chi- Square, Fishers Exact, Kruskal-Wallis and Multiple Logistic Regression tests were used, adopting a significance level of 5%. The results showed statistical difference between the groups: CG presented a higher; number of healthy teeth (p=0.038) and teeth without changes in enamel (p=0.005) when compared to OG. When comparing CG and EG after the second evaluation (T1), statistically significant difference was verified in relation to DMFT (p<0.0001), restored teeth (p<0.0001) and lesion area (Ws) (p=0.045), being the worst conditions found in OG. It was observed that there was a significant difference between mineral loss and the quantity dental biofilm by fluorescence between groups, but not between T0 and T1. In view of the results, the authors concluded that patients with metabolic syndrome, compared to eutrophic, have a higher number of caries lesions before and after bariatric surgery. In the present study, bariatric surgery did not influence in caries lesion progression, however, highlights the necessity to a continuous multi professional approach to acting in the prevention of diseases and dental care.(AU)

ABSTRACT Purpose proper chewing and swallowing functions have great importance in general health, since it potentially affects food’s digestion and the subject’s nutritional status. The aim was to assess the validity and reliability of the self-applied Quality of Masticatory Function Questionnaire (QMFQ) in a convenience (non-referred) sample of Brazilian adolescents divided into three groups: control (n=57), dental caries (n=51) and malocclusion (n=54). Methods caries and malocclusion were evaluated using DMF-S index (number of decayed, missing and filled primary and permanent surfaces) and Index of Orthodontic Treatment Need, respectively. The QMFQ comprises 26 items regarding frequency and intensity of chewing problems with five domains: Food-Mastication, Habits, Meat, Fruits and Vegetables. Data were submitted to descriptive statistics, Kolmogorov-Smirnov and Chi-square tests. Psychometric evaluation included measures of reliability (internal consistency - Cronbach's alpha and matrix of correlation) and discriminant validity (Kruskal-Wallis/Dunn post-test). Results the instrument showed satisfactory internal consistency, with significant positive correlations between the domains scores except between Habits and Vegetables. Total scale Cronbach's alpha coefficient was 0.87 and the coefficient did not increase significantly with the removal of each domain. Significant differences were found between controls and caries group in Food-Mastication, Meat and Fruits scores. Caries group also showed higher median values in Food-Mastication and Fruits than the malocclusion group. Conclusion the Quality of Masticatory Function Questionnaire showed acceptable properties regarding internal consistency, reliability, and discriminant validity in evaluating the impact of caries on the perception of the masticatory function quality of Brazilian adolescents.

The aim of this study was to analyze the incidence of caries, in relation to maternal risk behaviors and clinical conditions representing different levels of sickle cell disease severity. A total of 295 children aged 6 to 60 months participated in this cohort conducted from August 2007 to December 2008. They were diagnosed and monitored by the referral service of the state. Interviews were made with families to identify sociodemographic variables, and an oral exam was performed to determine dental caries. The SRQ (Self Report Questionnaire) scale was used to diagnose the presence of common mental disorders, and the CAGE (Cut down, Annoyed, Guilty and Eye opener) was applied to determine abusive use of alcohol. The absolute and relative frequencies of the variables of interest were analyzed by Chi-square and Mann-Whitney, with a 5% significance level. The incidence variables were analyzed according to the logistic regression model, with a confidence interval of 95%. Caries incidence (1.98; SD = 4.68) was higher in the HbSS genotype. There was a statistically significant association between caries incidence and both abusive use of alcohol (32.43%, RR = 1.99; 1.05-3.78; 95%CI) and common mental disorders (8.77% RR = 0.37; 0.15-0.93; 95%CI). There was also an association between caries incidence and maternal risk behavior, indicating that the care network should be expanded to include patients with sickle cell disease.

Abstract This study evaluated the impact of molar-incisor hypomineralization (MIH) on oral health-related quality of life (OHRQoL) according to the perceptions of schoolchildren and their parents/caregivers. This cross-sectional study consisted of a sample of 594 schoolchildren between 11 and 14 years of age and their parents/caregivers who answered the questionnaires CPQ11-14ISF:16 and P-CPQ, respectively. The main independent variable of this study was MIH of the schoolchildren. Experience of dental caries, malocclusion, and socioeconomic status were treated as confounding variables. Statistical analysis used descriptive analysis and Poisson regression with robust variance. The prevalence of MIH was 18.9%. The overall P-CPQ score ranged from 0 to 35 (average = 7.26 ± 6.84), and the overall CPQ11-14ISF:16 score ranged from 0 to 47 (average = 11.92 ± 7.98). Severe MIH was associated with a greater negative impact of the "functional limitation" domain (RR = 1.41; 95%CI = 1.01-1.97), according to parents'/caregivers' perceptions. Severe MIH was associated with a greater negative impact of the "oral symptom" domain (RR = 1.30; 95%CI = 1.06-1.60) and functional limitation domain (RR = 1.42; 95%CI = 1.08-1.86), according to the schoolchildren's perceptions. Schoolchildren with severe MIH had a greater negative impact on the oral symptom and functional limitation domains than those without MIH. According to parents'/caregivers' perceptions, schoolchildren with severe MIH had a greater negative impact on the functional limitation domain than those without MIH.

This study assessed the association between oral health knowledge and practices of pregnant Saudi women selected from visitors to a government hospital in Dammam, Saudi Arabia in 2014. Women answered questions on oral health knowledge during pregnancy and knowledge of infant oral health. Most women [> 70%] knew that dental caries in children can be prevented, that pregnancy affects oral health and that dental treatment during pregnancy can negatively affect infants. Most women [> 80%] performed oral hygiene procedures but only 18% regularly visited the dentist. In a regression analysis, oral health knowledge was not significantly associated with reported oral hygiene practices. Women who visited the dentist regularly were more likely to know how to prevent caries in children, and that dental treatment during pregnancy and infant health were associated

A saúde bucal é indispensável ao sucesso da reabilitação estética e funcional da criança com fissura labiopalatal. O objetivo desta pesquisa foi conhecer como a doença cárie se expressa em pacientes tratados desde os primeiros dias de nascido no Ambulatório de Prótese Buco Maxilo Facial da FOUSP. Um único pesquisador examinou 255 crianças portadoras de fissura labiopalatal na faixa etária de 6 a 72 meses, das quais 116 eram do gênero feminino e 139 do masculino, classificadas em 145 crianças com Fissura transforame incisivo, 36 com Fissura pré-forame incisivo e 74 com Fissura pós-forame incisivo. O exame clínico, visual e tátil, realizou-se após profilaxia dental, sob luz artificial por meio de espelho clínico e sonda IPC quando necessária. A cárie dentária foi diagnosticada pelos critérios da OMS, a severidade de acordo com a FDI/WHO. A análise estatística utilizou o teste Qui-Quadrado ajustado pela estatística de Fisher ou pela correção de Yates. As 35 crianças acometidas por cárie dentária corresponderam a 13,8% da amostra. Não houve associação da prevalência de cárie com os tipos de fissura (p=0,587) e com o gênero (p=0,736), mas o aumento da idade mostrou relação estatisticamente significante (p=0,001). O ceo-d médio na amostra transversal foi 0,34, com o score de severidade muito baixa. Concluiu-se que portadores de fissura labiopalatal, acompanhados desde o nascimento, apresentaram baixa prevalência de cárie dentária com severidade muito baixa...


Oral health is essential to the success of aesthetic and functional rehabilitation of children with cleft lip and palate. The objective of this research was to explore in greater depth how caries are expressed in patients treated since the early days of FOUSP’s Prosthetic Maxillofacial Ambulatory care. A single researcher examined 255 children with cleft lip and palate between the ages of 6 and 72 months of which 116 were female and 139 were male, which classified 145 children with cleft lip and palate, 36 with cleft lip fissure and 74 with cleft palate fissure. Visual and tactile clinical examinations were held after dental prophylaxis under artificial light through clinical mirror and CPI probes when necessary. Dental caries were diagnosed by WHO criteria, severity according to the FDI / WHO. The tabulated data was statistically analyzed using the chi-square test adjusted for the Fisher statistic or the Yates correction. The 35 children were affected by dental caries which accounted for 13.8% of the sample. There was no association between caries prevalence with cleft types (p = 0.587) and gender (p = 0.736), but increasing age showed a statistically significant correlation (p = 0.001). The mean dmft in the cross-sectional sample was 0.34, with the severity score very low. The aim of this study was to assess the prevalence of early childhood caries (ECC) in children and investigate the influence of sociodemographic variables, quality of oral hygiene and child-related aspects. A cross-sectional study was carried out with 593 children aged three to five years. Data were collected through clinical examinations and interviews with parents. Interviews with parents of the children were conducted to acquire information on sociodemographic aspects, breastfeeding, bottle feeding and harmful oral habits. Statistical analysis involved the chi-square test and the Poisson regression. The prevalence of ECC was 53.6%. The occurrence of ECC was greater among children with unsatisfactory oral hygiene (PR: 2.95; 95% CI: 2.42-3.60) and those from a family with a lower monthly household income (PR: 1.62; 95% CI: 1.24-2.10). In conclusion, unsatisfactory oral hygiene and monthly income exerted an influence on the occurrence of ECC among preschoolers.

The objective of this study was to evaluate in vitro the effect of a low fluoride toothpaste (450 µgF/g, NaF) combined with calcium citrate (Cacit) and sodium trimetaphosphate (TMP) on enamel remineralization. Bovine enamel blocks had the enamel surface polished sequentially to determine the surface hardness. After production of artificial carious lesions, the blocks selected by their surface hardness were submitted to remineralization pH cycling and daily treatment with dentifrice suspensions (diluted in deionized water or artificial saliva): placebo, 275, 450, 550 and 1,100 µgF/g and commercial dentifrice (positive control, 1,100 µgF/g). Finally, the surface and cross-section hardness was determined for calculating the change of surface hardness (%SH) and mineral content (%∆Z). Fluoride in enamel was also determined. The data from %SH, %∆Z and fluoride were subjected to two-way analysis of variance followed by Student-Newman-Keuls's test (p<0.05). The mineral gain (%SH and %∆Z) was higher for toothpastes diluted in saliva (p<0.05), except for the 450 µgF/g dentifrice with Cacit/TMP (p>0.05). The 450 Cacit/TMP toothpaste and the positive control showed similar results (p>0.05) when diluted in water. A dose-response was observed between fluoride concentration in toothpastes and fluoride present in enamel, regardless of dilution. It was concluded that it is possible to enhance the remineralization capacity of low F concentration toothpaste by of organic (Cacit) and inorganic (TMP) compounds with affinity to hydroxyapatite.


O objetivo do presente trabalho foi avaliar in vitro o efeito de um dentifrício com reduzida concentração de fluoreto (450 µgF/g, NaF) associado ao citrato de cálcio (Cacit) e trimetafosfato de sódio (TMP) na remineralização do esmalte. Blocos de esmalte bovino tiveram sua superfície de esmalte polida seqüencialmente para determinação da dureza de superfície. Após o desenvolvimento de lesões artificiais de cárie, os blocos selecionados através da dureza de superfície foram submetidos a ciclagem de remineralização e tratamento diário com suspensões de dentifrícios (diluição em água deionizada ou saliva artificial): placebo, 275, 450, 550 e 1.100 µgF/g e com dentifrício comercial (controle positivo, 1.100 µgF/g). Ao término, determinou-se a dureza de superfície e em secção longitudinal, para cálculo da variação da dureza de superfície (%SH) e do conteúdo mineral (%∆Z). O fluoreto presente no esmalte também foi determinado. Os dados de %SH, %∆Z e fluoreto foram submetidos a análise de variância a dois critérios seguido pelo teste de Student-Newman-Keuls (p<0,05). O ganho mineral (%SH e %∆Z) foi maior para os dentifrícios diluídos em saliva (p<0,05), exceto para os dentifrícios 450 µg F/g com Cacit/TMP (p>0,05). Os dentifrícios 450 Cacit/TMP e controle positivo apresentaram resultados semelhantes (p>0,05) quando diluídos em água. Uma relação dose-resposta foi observada entre a concentração de fluoreto nos dentifrícios e o fluoreto presente no esmalte, independente da diluição. Concluiu-se que é possível melhorar a capacidade de remineralização de dentifrícios com reduzida concentração de fluoreto pela adição de compostos orgânico (Cacit) e inorgânico (TMP) com afinidade a hidroxiapatita.

La investigación ha estimulado un cambio de los enfoques tradicionales de los tratamientos, haciendo hincapié en la amplia variación de la susceptibilidad individual a las enfermedades periodontales y pérdida de los dientes. Hace más de 40 años se estableció científicamente la relación causal entre la biopelícula y la gingivitis. En los últimos años ha surgido evidencia, desafiando los conceptos tradicionales en la cual se basan abordajes para el tratamiento de enfermedad periodontal. La comprensión genética está comenzando a explicar las diferencias del huésped a esta susceptibilidad y también la posibilidad de respuesta satisfactoria al tratamiento. Ahora se confirman los efectos del estrés psicológico y el tabaco. Hoy el sueño de regeneración periodontal con inserción del tejido conjuntivo es una realidad. También se proporciona a los clínicos medios más eficaces de detección, prevención y tratamiento de la periodontitis, algunos ya actualmente disponibles; un enfoque más acorde de ser los médicos de la cavidad bucal. Nos acercaremos a enfermedades bucales utilizando un modelo más biológico de la gestión de la salud-enfermedad. Research is stimulating our traditional approaches to treatment needs by emphasizing the wide variation in individual susceptibility to periodontal diseases and tooth loss. Over 40 years ago, the causative relationship between dental plaque and gingivitis was scientifically established. In recent years, evidence has emerged challenging the traditional concepts of periodontal disease upon which approaches to treatment are based. Genetic understanding is beginning to explain host differences in this susceptibility and also the potential for satisfactory treatment response. The effects of psychological stress and tobacco smoking are now confirmed. Today the dream of periodontal regeneration with connective tissue attachment is a reality. It do provide clinicians with more effective means of detection, prevention, and treatment of periodontitis, some of them are currently available; an approach more befitting physicians of the oral cavity. We will approach oral diseases using a more biological model of health- disease management. The objective of this study is to update us, based on the existing evidence, principles of periodontal therapy and its relationship with the clinical comprehensive dentistry

A amostra foi constituída por 21 pacientes obesos submetidos à cirurgia bariátrica e acompanhados após 3 (3M), 6 (6M) e 12 (12M) meses, e por 16 pacientes não obesos submetidos à colecistectomia (GC). As condições bucais avaliadas foram: cárie (CPOD e ICDAS II), doença periodontal (IPC), desgaste dentário (IDD) e o fluxo salivar. Os questionários BAROS e OIDP foram utilizados para verificar a qualidade de vida e o impacto da saúde bucal. Os testes de Mann-Whitney, Friedman, Wilcoxon e Coeficiente de Correlação de Spearman foram aplicados (p<0,05). O CPOD médio foi de 18,0±3,4; 18,7±3,4; 18,9±4,6; 19,0±4,5 e 14,8±7,2 para PRÉ, 3M, 6M, 12M e GC, respectivamente (p>0,05). Após a cirurgia bariátrica os pacientes apresentaram aumento significativo na incidência de lesões cariosas iniciais (PRÉ= 28,6%; 3M= 4,8%; 6M=42,8%; 12M= 71,4%; e GC=75,0%). A condição periodontal não se alterou após a cirurgia bariátrica (bolsa periodontal- PRÉ= 57,1%, 3M= 52,4%; 6M= 38,1%; 12M= 76,2%; GC= 31,2%) (p>0,05). Todos os pacientes apresentaram algum grau de desgaste dentário, sendo que os valores IDD foram PRÉ=1,3±0,2; 3M=1,3±0,3; 6M=1,4±0,3; 12M=1,4±0,3 e GC=1,5±0,3. Incidência e severidade de desgaste dentário aumentaram após a cirurgia bariátrica (p= 0,000). A média do fluxo salivar não sofreu alteração após a cirurgia bariátrica e foi maior quando comparada ao GC (p>0,05). The aim of this study was to evaluate oral health changes such as dental caries, periodontal disease, dental wear and salivary flow in bariatric patients. The sample consisted on 21 obese patients who had been submitted to bariatric surgery and followed up for 3 (3M), 6 (6M) and 12 (12M) months and also on 16 patients not obese submitted to cholecystectomy (CG). Oral conditions evaluated: dental caries (DMFT and ICDAS II), periodontal disease (CPI), dental wear (DWI) and salivary flow. Such BAROS as OIDP surveys were utilized to verify quality of life and oral health impact. Mann-Whitney, Friedman, Wilcoxon and Spearman`s Correlation Coefficient tests were applied (p<0.05). The mean of DMFT was 18.0±3.4; 18.7±3.4; 18.9±4.6; 19.0±4.5; and 14.8±7.2 to PRE, 3M, 6M, 12M and GC respectively (p>0.05). After bariatric surgery, patients presented significant increase in the incidence of initial carious lesions (PRE= 28.6%; 3M= 4.8%; 6M=42.8%; 12M= 71.4%; and GC=75.0%). Periodontal condition did not change after bariatric surgery (periodontal pocket PRE= 57.1%, 3M= 52.4%; 6M= 38.1%; 12M= 76.2%; GC= 31.2%) (p>0.05). All patients presented certain degree of dental wear, considering that the DWI values were PRE=1.3±0.2; 3M=1.3±0.3; 6M=1.4±0.3; 12M=1.4±0.3 and GC=1.5±0.3. Incidence and severity of dental wear have increased after bariatric surgery (p= 0.000). The mean of salivary flow did not change after bariatric surgery and was the same when compared to GC (p>0.05). There was correlation only between salivary flow and dental wear in 12M (r= -0.458; p<0.05). According to BAROS protocol, patients submitted to bariatric surgery reported that quality of life has improved after surgical procedure and significant difference was verified only among 3M and 12M periods (p=0.003). It was verified that oral health impact on patients quality of life after bariatric surgery is minor (PRE= 19.4±31.4; 3M= 5.6±9.1; 6M= 5.4±11.3; 12M= 11.8±23.2) (p>0.05)...

The sample consisted on 21 obese patients who had been submitted to bariatric surgery and followed up for 3 (3M), 6 (6M) and 12 (12M) months and also on 16 patients not obese submitted to cholecystectomy (CG). Oral conditions evaluated: dental caries (DMFT and ICDAS II), periodontal disease (CPI), dental wear (DWI) and salivary flow. Such BAROS as OIDP surveys were utilized to verify quality of life and oral health impact. Mann-Whitney, Friedman, Wilcoxon and Spearman`s Correlation Coefficient tests were applied (p<0.05). The mean of DMFT was 18.0±3.4; 18.7±3.4; 18.9±4.6; 19.0±4.5; and 14.8±7.2 to PRE, 3M, 6M, 12M and GC respectively (p>0.05). After bariatric surgery, patients presented significant increase in the incidence of initial carious lesions (PRE= 28.6%; 3M= 4.8%; 6M=42.8%; 12M= 71.4%; and GC=75.0%). Periodontal condition did not change after bariatric surgery (periodontal pocket PRE= 57.1%, 3M= 52.4%; 6M= 38.1%; 12M= 76.2%; GC= 31.2%) (p>0.05). All patients presented certain degree of dental wear, considering that the DWI values were PRE=1.3±0.2; 3M=1.3±0.3; 6M=1.4±0.3; 12M=1.4±0.3 and GC=1.5±0.3. Incidence and severity of dental wear have increased after bariatric surgery (p= 0.000). The mean of salivary flow did not change after bariatric surgery and was the same when compared to GC (p>0.05). There was correlation only between salivary flow and dental wear in 12M (r= -0.458; p<0.05). According to BAROS protocol, patients submitted to bariatric surgery reported that quality of life has improved after surgical procedure and significant difference was verified only among 3M and 12M periods (p=0.003). It was verified that oral health impact on patients quality of life after bariatric surgery is minor (PRE= 19.4±31.4; 3M= 5.6±9.1; 6M= 5.4±11.3; 12M= 11.8±23.2) (p>0.05)...

The aim of this study was to evaluate effect of bleaching agents on sound enamel (SE) and enamel with early artificial caries lesions (CL) using confocal laser scanning microscopy (CLSM). Eighty blocks (4 x 5 x 5 mm) of bovine enamel were used and half of them were submitted to a pH cycling model to induce CL. Eight experimental groups were obtained from the treatments and mineralization level of the enamel (SE or CL) (n=10). SE groups: G1 - unbleached (control); G2 - 4% hydrogen peroxide (4 HP); G3 - 4 HP containing 0.05% Ca (Ca); G4 - 7.5% hydrogen peroxide (7.5 HP) containing amorphous calcium phosphate (ACP). CL groups: G5 - unbleached; G6 - 4 HP; G7 - 4 HP containing Ca; G8 - 7.5 HP ACP. G2, G3, G6, G7 were treated with the bleaching agents for 8 h/day during 14 days, while G4 and G8 were exposed to the bleaching agents for 30 min twice a day during 14 days. The enamel blocks were stained with 0.1 mM rhodamine B solution and the demineralization was quantified using fluorescence intensity detected by CLSM. Data were analyzed using ANOVA and Fisher’s tests (α=0.05). For the SE groups, the bleaching treatments increased significantly the demineralization area when compared with the unbleached group. In the CL groups, no statistically significant difference was observed (p>0.05).The addition of ACP or Ca in the composition of the whitening products did not overcome the effects caused by bleaching treatments on SE and neither was able to promote remineralization of CL.


O objetivo deste estudo foi avaliar o efeito de agentes clareadores no esmalte sadio (ES) ou esmalte com lesão inicial de cárie artificial (LC), utilizando microscopia confocal laser de varredura (CLSM). Oitenta blocos (4 x 5 x 5 mm) de esmalte bovino foram usados, sendo que 40 destes foram desmineralizados com ciclagem de pH para induzir a LC. Oito grupos experimentais foram obtidos a partir dos tratamentos e condição do esmalte (ES ou LC), com n=10: Grupos ES: G1 - sem tratamento (controle); G2 - peróxido de hidrogênio 4% (PH4); G3: PH4 contendo 0,05% cálcio (Ca); G4 - peróxido de hidrogênio 7,5% (pH 7,5) contendo fosfato de cálcio amorfo (ACP). Grupos LC: G5 - não clareado; G6 - pH 4; G7 - pH 4 Ca; G8 - pH 7,5 ACP. Os grupos G2, G3, G6 e G7 foram tratados com o gel clareador por 8 h/dia durante 14 dias, enquanto as amostras dos grupos G4 e G8 foram submetidas ao agente clareador por 30 min/duas vezes ao dia, durante 14 dias. Os blocos de esmalte foram corados com solução de Rodamina B e a área fluorescente de desmineralização foi quantificada utilizando CLSM. Os dados foram submetidos a ANOVA e teste de Fisher (p<0,05). Para ES, os tratamentos clareadores aumentaram significativamente a área de desmineralização quando comparado com os grupos não clareados, entretanto, para LC não foi observado diferença estatística significante entre os grupos. A adição de ACP e Ca na composição dos géis clareadores não anulou os efeitos dos tratamentos clareadores no ES, assim como não teve capacidade de remineralizar o LC.

RESUMO

Dental decay is known as a multifactorial disease in which diet assumes an important significance. The consumption of carbohydrates in certain periods of time allows the production of acids by the bacteria present in the biofilm with the consequent mineral lost from the teeth substrate. Actually there are many studies that have considered the effect of some diet components on the oral health. The sucrose is the most cariogenic disaccharide commonly present on the family diet. The aim of this review is to find in the literature the relationship between nutritional aspects, alimentary habits and decay disease. This paper concluded that a negligent alimentary practice contributes with dental decay development. Alimentary habits and sociocultural family conditions are strongly related. That is why positive models must be adopted by the parents and early transferred to their children becoming those healthy habits persistent into their lives


La caries dental es una enfermedad considerada multifactorial en la cual la dieta cumple un papel importante para su desarrollo. El consumo de carbohidratos en diversos períodos durante el día permite la producción de ácidos capaces de causar la pérdida mineral del substrato dental (desmineralización). En la actualidad, diversos estudios consideran el efecto de ciertos componentes de la dieta sobre la salud bucal. La sacarosa, considerada el disacárido más cariogénico presente frecuentemente en la dieta familiar colaboran con el desequilibro de salud bucal. El objetivo de esta revisión fue buscar en la literatura datos que fundamenten la relación de las variables nutrición y dieta con la enfermedad. Se concluyó que la práctica alimenticia negligente, con nutrición y dieta desfavorables, aportan para el desarrollo de la enfermedad. Existe una fuerte relación entre hábitos alimenticios y condiciones socioculturales familiares. Siendo así, modelos positivos deben ser adquiridos y transferidos precozmente de los padres a los hijos, tornando esos hábitos persistentes en la vida de los niños y contribuyendo positivamente para su salud
